Appointments
Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and appreciate your cooperation. Missing 3 appointments in a one year period will be grounds for dismissal from the practice. A $25 fee will be charged for missed appointments.
Prescription Refills
We ask that you bring all prescription medication(s) with you to your appointment, even if it was prescribed by another physician. Please request all refills of continuous medicines during your appointment.
In the event of your prescription expiring prior to your next appointment, contact us. Generally, we can request a prescription be refilled at your pharmacy within 24 hours to 48 business hours.
